Who are the Patient champions?
Patient Champions are volunteers who work with the Waterside PCN GP practices to support patients through community activities.
We promote healthy lifestyles through signposting patients to relevant services and information and provide opportunities for individuals to make positive behavioural changes through activities aimed at improving their health and wellbeing.
